	Please download ES02 User Account Application Form – Heavy Vehicle Park Operator or ES03 User Account Application Form at OneMotoring > Forms > User Account Application Forms and upload the completed form via www.lta.gov.sg/feedback to reset your password. You will receive your new system-generated password at your registered address within 5 working days.
